marie123: tapering trazodone

Recommended Posts

I have been taking Trazadone since January 2014. I started with .75 Traz and have been slowly tapering the past six months. I am now down to .38 Traz after making a cut 2 nights ago from .45 Traz.. I take it at night to sleep along with 7.5 Remeron.

I have been doing well with sleeping on the .45 which I have been holding the past 2 months. Last night my sleep was very light and fitful after the cut.  I doubt I got that much sleep. Also, this October I will be on year off Ambien 10 mg.

I am looking for some encouragement from folks who have been down this road before with Trazadone. Any success stories out there?

How did you feel along the way with tapering? How was your sleep?

How did you feel one week off Traz, three weeks off, a month off?

Hi Marie-- welcome to the group. I'm so glad you found us.  Sorry to hear that your latest drop is causing some trouble.  We do recommend that people limit their reductions to 10% of their previous dose to minimize the WD symptoms.  Your cut was a bit bigger than that, which is probably why you're having some symptoms.  We have a thread dedicated to tapering traz that you should take a look at;

Tips for tapering off trazodone (Desyrel)

What is withdrawal syndrome?

Why taper by 10% of my dosage?

If you would please add a signature block with all of your drug history it would be really helpful.

Please put your withdrawal history in your signature

20 years on Paxil starting at 20mg and working up to 40mg. Sept 2011 started 10% every 6 weeks taper (2.5% every week for 4 weeks then hold for 2 additional weeks), currently at 7.9mg. Oct 2011 CTed 15oz vodka a night, to only drinking 2 beers most nights, totally sober Feb 2013.

Since I wrote this I have continued to decrease my dose by 10% every 6 weeks (2.5% every week for 4 weeks and then hold for an additional 2 weeks). I added in an extra 6 week hold when I hit 10mg to let things settle out even more. When I hit 3mgpw it became hard to split the drop into 4 parts so I switched to dropping 1mgpw (pill weight) every week for 3 weeks and then holding for another 3 weeks.  The 3 + 3 schedule turned out to be too harsh so I cut back to dropping 1mgpw every 4 weeks which is working better.

Final Dose 0.016mg.     Current dose 0.000mg 04-15-2017

Hi everyone,

I've just restarted my Trazadone taper. I started with .75 Traz in January 2014 and have tapered down to .45 Traz without too much trouble. I also take 7.5 Remeron. I take both for sleep. I have been off Ambien one year in October.

I have been dry cutting and 2 nights ago went down to about .38 Traz. I might have made too large a cut as I had a bad night's sleep and have some anxiety today. I'll make the same cut tonight and if I don't get some decent sleep, I may updose back to .45 Traz. I may have to rethink my tapering strategy.

Anyone drycut Traz to the very end? How did you taper your Trazadone and how are you now? Thanks.

Just a quick update. I am currently down to 33 mg Trazadone. I make small cuts each month. Anyone else out there tapering maybe we can share our experiences. So far when I cut I feel it 3 days later, little anxiety and moodiness. My sleep gets more disturbed for about a week, but then it all evens out. I wonder how quick I can taper this med.I

It doesn't seem you could go faster, unless you want to put up with an increase in sleep disturbance.

Hey Marie - it sounds like you are doing very well, having come off most of your drugs by now.  I was on Trazadone in the 80's (I think.  Those times are very fuzzy.  It might have been my husband at the time.   )

I would keep going - slowly.  I like that you are leaving an entire month between tapers, and if the only symptom you are having is a few nights of disrupted sleep, then that isn't too bad.

I wouldn't speed it up, though, because as you get down to lower doses, the tiny changes you make can have greater effect.  There are people here who have been sensitive to the tiniest crumbs of their psych drug.  That is why a liquid taper is often easier - those tiny adjustments are easier to make when you dilute your drug and measure it out in a syringe.  Being consistent is important when your doses are so tiny.

So take care, and listen to your symptoms.  Wait for them to settle, and then wait a couple of more weeks before you taper again.

The drugs are kind of like quicksand, if you thrash about too much, you will sink faster.  Going slow and steady is what saves your life!

I have a "good news" update. I have dropped to 12.5 mg Trazadone and feeling pretty good. When I went under 25 mg the mild depression lifted, and the lower I go the more awake and more like my old self I feel. I have been speeding up the tapering to about every 2 weeks and it's working for me. What's a bit weird is that I am sleeping better now. The only thing I can think of is the Remeron 7.5 mg is having a more intense effect on me, now that I am taking only a very small dose of Traz. It's all very interesting, maybe I'll even be a Traz withdrawal success story.

Hello everyone. It's good to hear positive news for those who are tapering. I was able to get down to only 9 mg Trazadone. A lot of the morning fog has lifted, memory has improved, and my thought processes are better. For instance, last year putting up my Christmas lights I could not figure how to put a few strings of lights together on my tree and get them all to light up. This year I was able to do it no problem. I have more energy also. I've been cutting a little faster than recommended, but will slow down a little now. I started the taper back in February of this year, and completed my Ambien taper 13 months ago. I notice when I make a cut I wake up more at night for a few days and then it goes back to normal. The second phase of the cut in week two I get mild anxiety, but sleep is fine. That passes too by the end of that week.

All in all, I feel positive about the rest of this taper and the upcoming taper (Mirtazapine). Good luck to all of us.

Hello folks. I AM FINALLY DONE WITH TRAZADONE! I jumped off at 4mg a few weeks ago. Some withdrawal, but nothing terrible. Thanks for all the support here.

Hi Marie,

I'm interested in your taper, as I am on amitryptiline, which is similar to trazadone. Not a lot of amitryptiline or trazadone users around anymore, really glad to hear that your taper went pretty smoothly. I tried trazadone once, but hated it and it didn't help me sleep at all. Did you dry taper all the way down?

Yes I did. I used a razor blade and did it by eye. I did not weigh at all. I'm sure it wasn't completely accurate, but I found it did not have to be with Traz. I just went slowly. I started out with a cut and then wait for a month. As I knew how the cuts would affect me, I became more confident and cut a little faster.

If your pill isn't real tiny, it's do-able dry cutting. Good luck.

Well I thought I was okay and got past the Trazodone withdrawal. I jumped New Year's Eve. In the middle of February I caught a stomach bug which required an antiobiotic. I was weak, nauseus, had the runs, etc. I completed the antibiotic and I still have no appetite and feel nauseus all the time. I am not sure if all of this is late withdrawal. It's almost three months since my last dose of Trazadone. I jumped off at 4 mgs. Anyone have nausea, loss of appetite, basically gastric issues from withdrawal of their AD and it finally stopped?

For all I know this could be an Ambien wave (17 months off). What does everyone think I should do? Is this Trazodone withdrawal so late? Should I reinstate at this point? Please help I just don't know what to do. Thanks.

I would not reinstate so far out. I would look into probiotics, antibiotics can really mess up your gut microbiome, and sometimes it takes quite a while to get that back in line. Probiotics will speed up that process a lot. Your stomach may still be adjusting to the lack of serotonin, so it is a bit sensitive. If you are still taking Remeron, that could also affect your gut.

Welcome back marie.

This absolutely could be protracted withdrawal syndrome from tapering then stopping the trazozdone

too fast.

"Posted 18 October 2015 - 09:09 AM

I am currently down to 33 mg Trazadone. I wonder how quick I can taper this med."

If you had followed the recommended 10% method , your taper would look something like this:

18 October 33mg

18 November 29 mg

18 December 26mg

18 January 23mg

18 February 20.5mg

18 March 18mg

If you were at 4mg on 31 January , you clearly went much faster than we suggest is safe.

Please read the thread on reinstatement here and come back with any questions

http://survivingantidepressants.org/index.php?/topic/7562-about-reinstating-and-stabilizing-to-reduce-withdrawal-symptoms/

Reinstating could save you many many months of suffering far worse symptoms than you have now.

The way you tapered is as good as stopping cold turkey , and you can see from other members how

this worked out for them.

At 3 months out , you still have a very good chance that the reinstatement will be successful.

Please could you add the details of your trazodone taper to your signature. Also , some doses

are in whole mg's , and some are written as 0.? I have assumed all your trazodone doses are meant

to be in whole mg's.

Sept. 2012 -decreased 90mg in 6months. Care taken over by Dr Lucire in March 2013 , decreased last 30mg at 2mg per week over 3 months. July 21 , 2013- last dose of Cymbalta

Protracted withdrawal syndrome kicked in badly Jan.2014 Unrelenting akathisia until May 2014. Voluntary hosp. admission. Cocktail of Seroquel, Ativan and mirtazapine and I was well enough to go home after 14 days. Stopped all hosp. meds in next few months.

July 2014 felt v.depressed - couldn't stop crying. Started pristiq 50mg. Felt improvement within days and continued to improve, so stayed on 50mg for 8 months.

Began taper 28 Feb. 2015. Pristiq 50mg down to 45mg. Had one month of w/d symptoms. Started CES therapy in March. No w/d symptoms down to 30mg.

October 2015 , taking 25mg Pristiq. Capsules compounded with slow-release additive.

March 2016 , 21mg

Thanks for your response Fresh. I know I went faster than recommended, but I thought I knew how each cut affected me. Yes all doses cut were in mgs. I felt good up until the middle of February.  I did not anticipate it to come back at me 2/3 months later. What dose should I re-instate at? I jumped at 4 mg. 1/1/16, not 1/31/16. I never weighed the dose, I just did it by eye. I have 100 mg pills.  I have a scale now so I can do a smaller dose if you think I should.

What Saraberra says makes sense too. I just got over a stomach bug, and my system might need time to settle. I don't know if that's it or I need to re-instate. It's so hard to know. Any advice Fresh? I'll update my signature soon. It's 4 am here and I couldn't sleep anymore thinking about all this. Thanks.

To be frank , I think your "stomach bug" is from withdrawal - nausea , vomiting , lack of

appetite and diarrhea are pretty standard. If it hasn't come yet , you're likely to be hit by

"the flu' " next - cold symptoms with achy joints and muscles , and sensitive skin.

I did the same thing in 2013 marie. I tapered over 3 months , and for the first few months

things were fine. Better than fine - I was energized , had loads of enthusiasm and ideas , my

mood was often euphoric. Around month 3 I became more irritable , quick to anger , cried very

easily. In month 6 akathisia started , and I was housebound and terrified for the next 5 months.

I needed carers to organize everything in my home , and it was all I could do to shower each day.

So , 11 months after stopping , I had my first ever hospital admission , got re-medicated , and

was able to go home and manage after 3 weeks.

In light of this , of course my advice is to save yourself a heap of suffering and reinstate.

(Sorry about the date mix-up - new years eve is 1 Jan.)

After 3 months , you'd want to start with 1-2mg and see how it affects you. This is enough for

many people.

You need to stay on the starter dose for 4 days minimum to assess the effect - it takes that long

to reach a steady state in your blood stream.

You can weigh a 100mg tablet and divide by 100 to figure out the weight of each mg.

Hi Fresh thanks for responding again. Today I woke up without nausea and i had an appetite. I was able to eat all meals/snacks. I'm still not 100% but improved so I think I'll wait to see how the next few days go. I just am not sure that I need to reinstate. This is just so scary because it's so unpredictable. I am taking culturelle probiotic and eating yogurt with active cultures to hopefully heal my gut. I still have some loose bowels but that could be from the antibiotic.

I will definitely reinstate if I feel no improvement, and I do understand that it should be done sooner than later. I know now that I should have gone slower. You can bet that when I taper Remeron I will be going at a snail's pace. It would help if I could find others who tapered Trazodone and encountered similar issues. Wow Fresh you sure went through a lot! I hope you are feeling much better now. Thanks again Fresh.

You might be right Sarabera. Who knows what all this was from? Stomach bug? Late withdrawal? I do know that my stomach is very sensitive now and needs to heal. I'm taking probiotics and have been for a week. Remeron for sure affects the gut. I think it has to do with stomach acid. It either slows stomach acid or produces more. I think it slows acid so food doesn't digest quickly enough. I might reinstate, I just have to see how the few days go. It's so hard to know what to do! Marie

This is so strange. I have been taking Culturelle probiotic lately. I forgot to take it during the day and took it late last night. It seemed to really rev me up and I could barely sleep. I had this same reaction when I started eating kefir a lot. I stopped that and my sleep returned. Well I think I am going to stop the probiotic. I am just so sensitive I guess. Anyone else have problems with probiotics?

I am no longer nauseous and my appetite is a bit better. Hopefully my sleep will calm down. Apparently my system was really thrown by all of this. Thanks everyone who has responded to my thread.

I stopped the Culturelle probiotic and was able to sleep. It wasn't perfect as I woke up a few times and had crazy dreams. I think also between the antibiotic/stomach bug I got messed up. I hope things get better and I'm not going to get worse because all of this was really late withdrawal from Trazodone. I'm going to take it day by day.  Psych meds sure do suck. Thanks everyone.

Glad to hear that you are doing better! Haven't heard of that reaction to a probiotic, but it's not unreasonable, as most of the neurotransmitters are secreted in the gut. Hopefully things will continue to improve.

Sanbera hi. I stopped the probiotic and of course my sleep came back. My sleep is fine, mood fine, but I still have nausea, especially in the morning. It lessens by noon but it's still there. I am beginning to think it is late Traz withdrawal. I'm pretty sure I had a bug, but I do think something was triggered. I am still considering updosing 1 or 2 mgs. I am going to be updating my signature so Fresh and others can see my Traz tapering. I will be deciding soon.

Hi Fresh. I have updated my signature to help understand how I tapered off of Trazodone. I am beginning to think this may be withdrawal unfortunately. If I reinstate should I reinstate at 1 or 2 mg? Am I still in the window of reinstating and how long before I know if it works and I feel better, maybe one or two weeks? I am sorry if I have so many questions but updosing is all new to me. My symptom is only nausea which is more intense in the morning, but lessens some as the day goes on. I am eating and not vomiting, just feel real nauseous. How does one know if they should just ride out the symptoms?

Thanks again. Marie

Hi Altostrata. Can you please also take a look at my Trazodone tapering history and tell me what you recommend. Should I reinstate 1 or 2 mgs? Also how do I know whether this is just a wave that will pass or that I should reinstate?

Thanks everyone.

Hi Marie, You may have seen on the other site how I was able to reinstate amitryptiline at a very small dose to head off stomach issues. My previous dosage was 12.5 mg., and I reinstated 2 mg. and felt relief almost instantly. Then I pared that down to .375 mg over the next few days and am continuing to do well. So you might consider doing .5 mg at a time and see if you get relief within a few hours. That's how I did it--.5 mg. then wait a few hours, then another dose. I'd hate to see you reinstate more than your jumping off dose. Amitryptiline is fairly water soluble, so that helps a lot, not sure about trazadone.

1975--first signs of depression

1981--started on imipramine (Tofranil) for IBS and depression

1983-1986--severe depression, rotated through several drugs, on MAOI for one year, eventually back to tricyclics

1986-1994--chronic low grade depression, on tricyclics

1994-96--severe depression, rotated through several drugs inc. Prozax, Effexor, etc..

1996-2013--chronic low grade depression, SAD, on amitryptiline usual dose 12.5-25mg

flurazepam (Dalmane) as needed for insomnia

2013--developed temazepam (Restoril) dependance for 2 months, tapered off over 1 month

started bio-identical progesterone 5 mg., depression has lifted completely to this day

March 2016--forced to c/t both amitryptiline and flurazepam, zolpidem not helpful

reinstated small dose (.5 mg) amitryptiline due to stomach issues and tapering w/titration

June 19th--jumped from amitryptiline--drug free!

Hi Sarabera. I think I cut way to fast at the end of my taper (Nov/Dec). In Dec. the taper went to 9 mg, 6 mg, 4 mgs then jumped. Do you take the A at night, but still did the .5 mg test during the day? That's interesting the way you did it. How long did you wait before reinstating? What's weird is that I felt fine for 2 1/2 months. What the hell you have to be a gd scientist to figure this stuff out.

I have the gemini scale now, so do you think I should try your test? Let me get this right, you tested it during the day by .5 by increments, waiting a few hours in between to see if you felt better? My only problem is nausea, I sleep fine, have no anxiety, my mood is good, etc.

Thanks,

Marie

Marie , when you compare previous regular doses of Sarabera's amitriptylene with your trazodone , you'll see that

1 to 2mg of traz. is equivalent to 0.5mg ami.

Your previous dose was 75mg for a year before tapering. Sarabera's dose was between 12.5 and 25mg for more than 20 years.

So you need to consider equivalency between medications.

Many people find symptom relief begins within hours of reinstating.

Take a look at the following list and see which symptoms you feel apply:

Hi Fresh. Thanks for responding to my post. I only have nausea w/ some loose bowels.  I would say the nausea is a 6 when I wake up and gradually thru the day gets better. The loose bowels have been improving daily. Right now at 5 pm it's about a 2/3. So you can see my dilemma whether or not to reinstate or not.

I think if I reinstate it will be 1 mg and see how it goes. That's interesting that many feel their symptoms improve right away. Fresh, since I take it at night, should I try .5 mg a bit earlier and see how I feel, then try .5 mg at bedtime. Maybe just to see if the .5 mg helps enough? Your help is very much appreciated.

Thanks again.

Here's the thing ... you need to stay on the same starter dose for 4 days in order assess the full effect.

It takes that long , 4 days , to reach a steady state in your blood stream.

If you split the dose , you won't know whether it's the first 0.5mg or the second 0.5mg that's making the difference.

I would keep it simple , one dose , 1mg.

Hi Fresh. I have been weighing my 100 mg Trazodone pills with the Gemini 20 Scale. I weighed 16 pills and then got an average weight of .6039375. I then divided that by 100 to get the weight of a 1 mg pill. The weight of the 1 mg pill is .00604. This would show up on the sale as .006 and would be 6 mg. This would be my 1 mg dose of Trazodone. Have I done this correctly?

How would I actually be able to taper 10% by weighing the doses since they are so tiny? I guess after I am stable I would have to do water titration, is this correct?

Thanks for all your help in this.

Oh dear , you're bringing my non-maths brain to the edge of it's capacity Marie :)

A pill with 100mg of active ingredient weight must weigh much more than 100mg , because they add fillers and buffers.

It can't be .6039375.

You have the theory correct of weighing the total pill weight then dividing by 100 to get the weight of 1mg of

active ingredient.
