Survivor1: sleeping after trazodone (and benzo)

Survivor1

Recommended Posts

Hello,

New member here.  Had been on Klonopin for 15 months for insomnia caused by hypothyroidism.  Been off K for 10 months now, with the help of 75 mg Trazodone for sleep.  About 2 months ago, T was losing its effectiveness and I decided it's time for a drug-free me.  I went from 75 mg to 50 with no problems; at 50, withdrawal hit hard.  Insomnia (of course), shakiness, anxiety, you name it.  I did a fast taper as I did not have it in me to go through another long taper after the K experience.

In 2 months, I went from 75, down by 1mg every night to 50, to 25 for 2 weeks, 12.5 for another 2 and finally off.  I have been off for only about 3 nights.

Withdrawal after quitting seems manageable so far, although it is too early to tell if it will get worse.  My main issue is with insomnia.  I feel if I can manage this, I will be OK, especially after the horrific K withdrawal.

My question for those who have used Trazodone:  how long did it take for sleep to return after quitting trazodone? Please include how long you had been using T and the dosage.

Thanks for your input and hope we all survive the trials of psychotropic drugs.

Hi Scallywag,

Thanks for checking in.  I was waiting a little while to post a more complete report.

As of now, things are turning for the better (I hope!). It was a VERY rough first three weeks, even flirted with reinstating a few times but decided against it since I know it can take a few weeks to feel the effects of reinstatement.  Somehow I hung in there.

My main symptoms, in order of severity were/are:  insomnia, bone-deep fatigue, muscle twitching, dizziness.  These past few days, the fatigue is much less, and I can sleep for about three hours on the days I do not take Seroquel (I alternate between 25 mg S and no sleep meds, to prevent having to discontinue a third psych med).  I hope this means life is slowly normalizing.  With the fatigue diminishing, I can actually do things around the house, and plan on getting some exercise regularly.

I will continue to post at regular intervals.

Thanks again for following up!

Great to hear that you are feeling better.  Be prepared that a wave of symptoms may arise for no apparent reason. Your CNS (central nervous system) is healing, rewiring to adapt to the absence of Trazodone.  I hope it's smooth sailing for you from here on out, but you can't take my hope to the bank of well-being as a guarantee. :(;)

Update ...

5 1/2 weeks later: fatigue completely gone, dizziness has significantly improved, muscle twitching and startle response have slightly improved.  But brutal insomnia and rapid heartbeat remain.

When I tapered from 75 to 50 mg, I began to lose handfuls of hair.  This subsided somewhat during the rest of the short taper, then restarted when i jumped off.  Still going on.  Have very thin hair right now.

This is the extent of my withdrawal so far.  I will consider myself healed when I can sleep at least 6 hrs per night consistently.  Hope this happens soon.

Hello Survivor1....are you still taking the seroquel?

Also, what has been your response to the 5-htp...good, bad, side effects?

Last, the Phosphatdylserine is that Seriphos?  If so what type of results have you seen from taking this?

The reason I ask is I have horrible issues with insomnia too and I just started seriphos last night and didn't really notice anything.

Glad some things are better for you!

Hi Downtongirl,

Yes, I am still taking Seroquel.  I take it on alternate nights (25mg), because I do not want to risk a third bout of withdrawal.  It works well, but does make me groggy in the mornings.  I will continue to take it for as long as needed.

For me 5-htp was useful during my taper from trazodone, but now I find it doesn't work.  I am convinced that my insomnia, heart palpitations, muscle twitching are the result of really elevated cortisol levels from quitting trazodone after a relative short taper, and only time will take care of that (and hopefully the Seriphos).  So if cortisol is not an issue for you, 5-htp may be worth a try.  It is recommended that it should be used for a few weeks only, so beware. I do not feel there was any negative side effect. I used 300 mg during taper and now 200mg.  I Will discontinue.

PS is indeed Seriphos.  Note that Seriphos has recently been re-worked and is now phosphatydyl S, and apparently is not at good as the original (phosphorylated S).  I find it does not help although it has been said that it takes a month or so to work and I have only been on it for a few days.  I plan to continue as it does not hurt!

Wish you the best. Insomnia is the worst!

Update...

So after worsening of insomnia, yes down to zero on nights without meds, and with frequent awakenings on nights with and unable to fall asleep after, I decided to reinstate.

I was very troubled by this because I toughed out 6 weeks of terrible withdrawal, the first three being insane.  I did have some breakthroughs but I could not function with the profound lack of sleep.

I reinstated 25 mg of trazodone last two nights, along with about 10 mg of seroquel.  I was able to sleep well.  I will give this reinstatement a week to work and if it doess, will try to chip away at the seroquel since I have been on it for only a short while.

I hope it works!

Hi Scallywag,

It's been 5 days since the 25 mg Traz reinstatement and I see some slight improvements:  Heartbeat is not thumping loudly in my chest as often and there are some periods of normal heart rate. Also, body twitches have decreased slightly (30%?).  However, insomnia, the Goliath, remains as is.  I would like to consider updosing to maybe 50mg. (Or 75mg which was my original dose, or use 5-htp which I had some success with in the past to induce sleep, other?)

Would appreciate your input!

Given that you're seeing improvement after 5 days, consider holding at 25 mg for another 7-10 days. You may find that your symptoms continue to ease, including insomnia.

I see I just repeated myself. Thanks for being both gracious and great about that. ;)

Do what you can to let go of ruminations "I'm not sleeping. I've got to sleep. OMG it's x am and I'm still awake." Lie down on your bed in a dark room with your eyes closed and do the best you can to rest. Even "microsleeps" and rest help.

So, after a few more days, I decided to updose to 50.  I have been sleeping for about 5 hours for the last 4 nights.  The alerting sensations, followed by anxiety, are so strong that they prevent me from falling asleep quickly and wake me up at 5 am despite the trazodone.

Any idea how long this symptom could last?  If I am unable to sleep for 6 hours consistently over the next 3 nights, I think I will updose again. Any thoughts?

Thanks!

Hi Survivor.  That is a doubling of your dose and then you are thinking of updosing again.    This could be problematic and reduce your chance of tapering off as you become more reliant on the Trazadone.  Often you need to give reinstatement a couple of weeks to properly ascertain the full effect.

It might be wise to hold now for a while to let the effects of the reinstatement catch up and let your system settle.

Sometimes , there is a certain level of discomfort that has to be gone through to get to the other side but if you can manage to do this then you can start to level off, stabilize and then start to slowly taper again.

Hi Ali,

Thanks for the response.

When I dc'ed klonopin, the alerting sensations lasted several months before subsiding. I am beginning to think that I may have to wait almost as long for them to subside again, as they are quite strong.  And so I am not too hopeful about the insomnia resolving after few weeks.  I foresee myself having to add a third drug (I have used 10 mg Seroquel in the recent past) every three days or so to augment Traz, which I do not like to do.

I can certainly wait another week to see how it goes.  Hopefully there is improvement.

Hello survivor1...just curious....why do  you wan to taper off of trazadone...is it giving you bad side effects?

Is your original insomnia due to your hypothyroidism?

Hi Downtongirl,

My original insomnia was from hypothyroidism which has been under control for about a year now.  Then, I was actually able to feel sleepy but couldn't fall asleep because I had severe hypnic jerks that literally started overnight when I was put on too high a dose of thyroid meds.  Even when I reduced the dosage the hypnic jerks remained.  So I was put on klonopin, which I used for 15 months.

I started trazodone to sleep after getting off the klonopin.  It has worked reasonably well, but there are many nights that it did not.  That and the fact that I do not have a psychiatric illness makes me want to come off, as I feel I should be able to sleep without meds.  Before the thyroid med fiasco, I was sleeping like a baby, 8 hours a night.

Trazodone also gives me side effects:  I cannot lose weight despite a healthy diet and exercise program, and I feel it is contributing to my hair loss (to be fair, I was losing hair before traz, but now I have proper thyroid and other hormone levels, great iron levels, off klonopin for almost a year, healthy diet,etc.).  I have a rash that is unresolved, and It also gives me the scariest dreams.

I am hoping the reinstatement works to give me decent sleep so I can try to taper (SLOWly this time!)

Wish you the best in your struggle!

  • 4 weeks later...

Hello,

Updating my situation ... it has been almost a month since I updosed Trazodone to 50.  I was able to sleep well, 6-7 hours for the first three weeks. Throughout this time I had every permutation of anxiety, alerting, muscle twitching, etc.  I was able to manage since I slept well.

About a week age, I seem to hit a wall (wave?).  Traz was/is not working as well.  I sleep about 4 hours, broken.  I have developed severe fatigue that I had only felt immediately discontinuing. Last night I had massive night flushes with sweats - this too I had only had immediately after the taper.  I had to add slivers of Seroquel (maybe 10 mg) occasionally to sleep 6 hrs.  Right now, I actually feel similar to the acute stage shortly after the rapid taper.  There are some positives:  alerting has reduced significantly, so has body jerks.  However, it seems that when one symptom improves, another worsens.

I am worried about not sleeping on Traz. I do not want to updose, and prefer not to use Seroquel.  I guess I am looking for reassurance that this is indeed a wave and sleep will return.

Is there anyone who has used both Traz and Ser simultaneously and came out"unscathed"?

If sleep does not improve over the next few weeks, should i just start tapering and just decide to use Seroquel?

Appreciate any thoughts!

Survivor -- it's best not to add another medication to deal with a symptom. I love getting a decent night's sleep and insomnia is one of the hardest symptoms for me.

Please add your history with Seroquel -- even if it's brief -- to your signature.

Scallywag,

Thanks for the reply.  I am not sure if your answer to both intermittent use of, and complete switching (last question above) to Seroquel is no.  Can you clarify?

Also, I am assuming this is probably a wave?

Neither: Intermittent use has a high risk of further destabilization.  Adding a second daily medication to deal with either a withdrawal symptom or an adverse effect of a medication is rarely a good.

It probably is a wave.  Now you know what one more flavor of your waves might look like -- one of those lessons that nobody really wants.

Hello,

It has been two months since my reinstating 50 mg Trazodone.  The last month has been very trying as I could not sleep more than 4 hours per night.  The other symptoms are mostly improved (although they take turns in recurring).  The lack of sleep after this long is severely impacting my life.  Yesterday I almost got in an accident while driving because I was so zombiefied.

I am seriously thinking of updosing to my original dose of 75, hoping to get 6 hrs of sleep some nights, and stabilizing for a few months.  (I believe that if I start to taper from 50, my insomnia will only get worse and I cannot manage on 4 hrs or less of sleep for over a year of tapering time.)  I have blackout shades, proper sleep hygiene, etc.

Please let me know what you think of updosing.  Also, if there are any supplements that others have used for sleep.  I have used melatonin and phosphatydylserine but with no success.

Thanks.

If you decide to updose, please do it in steps allowing 2-3 weeks before the next step up. You may find that you do well on a dose between 50 mg and 75 mg.

Do you think that updosing will make it harder for me to taper eventually?

Also, if I were to white knuckle it through on 50 mg, is it too early to think of beginning to taper now?  Recommend any sleep supplements to add to the T?

Starting to taper from a higher dose means that your taper will be longer. It's almost impossible to predict accurately whether it will be harder.

We don't recommend reducing dose when you have symptoms that you find intolerable.

Please keep notes on paper about your symptoms and about when you take your dose. Here's a post about the kind of notes that are most helpful: Take notes of doses and symptoms

I am beginning to suspect that I am getting tolerant of Trazodone.  Before the rapid taper, I was on 75 mg and initially was getting about 7 hrs of sleep.  It slowly deceased to 5.5.  This decrease was what prompted me to get off it (too quickly of course).  After I reinstated and got up to 50, I slept well for 6-7 hours for about a month, then I noticed the decrease beginning.  By the second month, I was sleeping for 4 broken hours a night.

I have seen a decrease in most of my symptoms, but if Traz has fizzled out and there is no more sleep improvement, would it be better to start reducing slowly?  (Frankly, I am so sleep deprived, I may updose just to get some relief.)  I would like to know what you all recommend when holding does not lead to more improvements, even though a few symptoms are intolerable right now.

Also, any recommendations for sleep supplements?

As always, thanks for your input.
