Tips for tapering Zoloft (sertraline)

Hi ChessieCat, and thanks.  

Cymbalta:  Currently 48.5 mg.  Previously 60 mg, 2011-Oct 2020.  

Taper Oct 11-13 (three day reduction of 30 mg relieved many Serotonin Toxicity symptoms but WD became too harsh). Oct 14- 24 (eleven day reduction of 10%, 54 mg).  Oct 25  (began second reduction of 10%, 48.5 mg; holding to stabilize and work on schedule).  

Zoloft  100 mg. 1993-2000 Stopped CT for pregnancy twice, then it pooped out.  Reinstated 2011-2020.    

Taper attempt:  Oct 17-24 (seven days at 87.5 mg was discontinued; reinstated to 100 mg). 

 

Buprenorphine: 8 mg (4 mg 2x day @ 4pm and 1am)  Atenolol: 25 mg (max 2x day; prn).  Xanax: .125 mg (max 3x day; prn; average 2x).  HRT: Transdermal weekly patch.  Melatonin OTC:  2.5 mg, sleep.  Magnesium Citrate OTC: 125 mg, dinner.

Oct-Nov 2020:  Discontinued Simvastatin 20 mg, Magnesium Glycinate, Vitamin D, and Chia Seed Extract. Growing sensitivities.

i'm sure one of the mods will be along soon, but since i've just seen this, i'l chime in with an opinion.

 

I think the main answer is it depends. If you do an ultra slow taper, you certainly minimise the chances of experiencing bad w/d symptoms, but to go as far as saying not experiencing any at all, i don't think so. Some people come off these drugs with little problems, whilst for others, it can be a real challenge.

 

Have a look at the tapering section of the forum, and look at the brassmonkey slide technique, it seems to be the most gentle way to taper and you can adjust it to be faster or slower.

Started Lexapro 5mg Mid March 2020

Came off Completely September 2020

Hospitalised september 2021

Slowly worked up to 50mg lamictal and 60mg Prozac

reduced to 20mg Prozac September 2021

Holding 20mg Prozac and 50mg Lamictal for a good while to stabilise. November 2021

psychiatrist made an abrupt taper then made me go on a even worse abrupt interruption resulting in terrible withdrawal symptoms.

 

reinstating clonazepam feb 2018 (4 days after stopping taking it)

improvement in symptoms after reinstating it and begging tapering thanks to benzobuddies help.

last dose clonazepam may 5 2020

terrible symptoms during tapering and after and no improvements so far

Yes, you can use tap water. If you have doubts about it, you can use filtered water or purified water. Or, you could buy pharmacy liquid base.

How do I get a 1mg dose from liquid Sertraline when 1ml of the solution is already a 20mg dose?

In summary, cycled through a number of SSRI/SNRI medications in 2010, stayed on 100mg Sertraline for 5 years followed by 5 years on 200mg.

Valium started in 2011-2018 up to doses of 16mg. Tapered off slowly over years. Difficult to distinguish withdrawal symptoms from the symptoms I was already experiencing due to Sertraline tolerance withdrawal

Briefly tried Lamotrigine and Bupropion in 2020.

Started withdrawing from Sertraline in August 2020. Came down by 50mg every 2-3 weeks. Med free since mid Sept 2020.

3 April 2021 - reinstated 1mg Sertraline, April 13th increased dose to 2mg Sertraline, April 16th increased to 5mg for one day. Stopped reinstatement on 19th April.

Main symptoms as of Nov 2020: Chronic headaches accompanied by feelings of deep despair, stomach ache/nausea, brain fog and memory issues, disturbed circadian rhythm.

End March 2021 symptoms: Extreme suicidal akathisia and restlessness, anxiety, insomnia, morning surges of panic, panic throughout the day. Unbearable.

April - suicidal akathisia, cortisol surges through the night, insomnia, shaking, GI issues

Supplements: Melatonin 2.5mg a night

You will need to dilute it with bottled or distilled water.

 

Measure 20mL water and add 1mL liquid sertraline to it.  Each 1mL = 1mg drug.  Or if you prefer you could add 1mL liquid sertraline to a larger measured quantity, eg to 40mL water then each 1mL = 0.5mg drug.

 

Cover and refrigerate what you do not use.  I do not know how long you could keep this.  Check with your pharmacist.

I wondered if I can ask a question? 

 

How do you challenge the GP/CCG decision to not issue Sertaline liquid on the grounds of cost? Surely this is not ethical prescribing. 

 

I am on 85mg and the prepare 10ml I dissolve a 25mg tablet in water and syringe out 10ml - not good but the best I can do. will be holding this dose for now until I restablise and get a new job as feel this is a contributing stressor. I work 3 days plus voluntary work. So pretty much a full time week for me.

 

One of my biggest gripes it being told no, when the NHS put me on this stuff in the first place without full informed consent. Mum is on 14 (Fourteen) different tablets in care so you can see psychologically how harmful I feel this is.

 

I have asked a few GPs now and get same answer. Has anyone tried challenging or appealing formally in the UK?

Help, I'm slightly below 50mg and am using 100mg pills (which I break into half, then shave one half of it). Should I switch to 50mg pills, while I still can? Would shaving them off be more precise? I imagine that half of a 100mg pill is slightly bulkier than a 50mg pill, and would therefore make tapering faster/more inaccurate.

2000: Paxil, low dose. 2001: Paxil, quickly tapered off. 2006: Zoloft, 25mg. 2007: Wellbutrin, low dose, discontinued after a week or a month. 2007-2009: Zoloft, increasing dose (up to 75mg). 2009-2011: Zoloft, failed withdrawal attempts (cold-turkey; fast tapering). 2009-2011: Trying out Cymbalta, Celexa, Prozac, Lexapro, Ambien, Ativan, and Xanax. 2012-2015: Zoloft, 100mg to 200mg. 2013: Trazadone for Zoloft-induced insomnia, 25mg to 50mg.  2015: Trileptal, dose?, withdrew; Bupropion, up to 200mg. 2016: Zoloft, 137mg, Bupropion, 150mg, Trazodone, 25mg.

05/21/16: Began Zoloft taper - 10% from 150mg - 137ish mg

06/23/16: Zoloft taper - 125mg

08/03/16: Bupropion XL taper - cut 150mg pill in crude half (mistake)

10/31: Zoloft successfully down to 100mg. Staying on 100mg for 2-3 months to let my brain rest.

11/09: Wellbutrin SR prescribed, 150mg once a day, for withdrawal.

12/11: Wellbutrin SR, twice daily - minus 18ishmg = 112.50mg

09/30/17: Off Wellbutrin SR successfully. Zoloft taper: 90ish mg (shaving it off). Trazodone: still at 50mg.

June 2018: Zoloft: 80ish (shaving off). Trazodone: 6mg. End of 2018: Off Trazodone.

01/20: Zoloft: 70ish?

08/20: Still around 80 or 70. Tapering by only 15 shaves each month.

04/2021: 50mg! 11/2022: 40ish mg

Can anyone provide me with a timeline and doseage for tapering off 12.5mg sertraline. Also is the hardest tapering done from 12.5 to 0mg? Or is it more difficult to go from something like 100mg to 50mg or 50 to 12mg. Thanks again

I can provide a spreadsheet that involves crushing the tablets and weighing the powder with a gem scale, mixing 15 days' worth of powder with water, then dosing daily using a medicine dropper.  If you want that, let me know and I will upload it. It is an excel file.

Lorazepam: (2014-15 1-1.5 mg lorazepam [aka Ativan] at night; 2016 to Nov 2018: 0.75 mg lorazepam at night; Dec 2018: divide dose into 3x a day. Jan 2019: taper 5% every 15 days; Held 9/2019 - 2/2020 at 0.35 mg). March 2020 started 2.5% taper every 15 days. Nov 2020 tapering 5% every 15 days. Dose divided into 2x a day. Now at 0.008 mg March 2023. Sertraline:  Jan 2017 25 mg: began 10% per month taper. Went on "exit strategy" at 0.084 mg Nov 2020 for 8 weeks, 0.042 for 4 weeks, 0.028 for 2 weeks starting 2/25/21, then 0.014 for 2 weeks, then .007 for a while, Done May 4, 2021. Unfortunately due to circumstances I restarted at 25 mg January 2022. New taper began January 2023.

Hi! I tried to find tips on Sertraline tapering with no success. It is for one of the people with whom I am creating the support group in UK. Any information would be highly appreciated. Many thanks, Julia (I hope I posted on the right place, if not apologies. )

2002 started Fluoxetine 20 ; increased gradually  to 40mg 

2004 diagnosed with ADD; prescribed methylphenidate

2004 later in the year  methylphenidate aggravated depression so discontinued   

2006 tried to taper  too quick from AD ( over 3 weeks!) and terrible WD; 

2006 later in the year doc changed to Citalopram (dont remember doze)

2009 (circa)  as Citalopram did nothing good so  back to 40 mg Fluoxetine 

2017 due to situational anxiety Pregablin prescribed terrible side effects 

201 8 tried again to taper fluoxetine 40 mg with   tough WD so back on it 

2020 changed to Escitalopram 20mg as no + effect of fluoxetine 

May 2023 prescribed Modafinil as fatigue intolerable

mid July 2023 started tapering - down to 18 mg daily so far so good

Can anyone tell me once I get down to the lower amounts ( I am at 23.3 mg right now) of Sertraline , is it possible to just keep cutting and weighing or do I have to go to the liquid.  Also, is there a chart somewhere that uses the 20mg per ml that shows comparison dosages .  If 20 mg is the  ml of the liquid does .1 ml mean there is .2 mg of sertraline.  I am terrible in math and I need to know if I should stay with cutting or if that is even feasible at lower dosages at all.  I am concerned for the future on coming towards the end.  

1991- Put on Prozac for menstrual problems 1991-2000: On Zoloft most of this time , 100mg . Max .2002 - Miscarriage . Proceeded to go off medication due to this. Got pregnant 2nd and was off medication for first trimester. Servere withdrawl problems went back on after first trimester. .2003-2010 Continued on Zoloft, Strattera 2010-2014 Lamictal Added. 2016 - Frontal Lobe Brain Tumor Discovered 2017 - Brain Surgery - stopped any attempts to taper. 2019 - 6 month polydrugged episode that included Lorazapeam,Rexulti,Pristiq,Gabapentin,Cymbalta,Ensam (after off antidepressants), Latuda, Abilify and Prozac. End of 2019 in October ended up in hospital with Akathesia and a slew of other symptoms. 2020-2021 Successfully tapered off: Lamictal, and all other meds except Lorazapam . Did a taper for 18 months off Lorazapam - Milk taper. 2021-current :Been tapering off Zoloft since February 2021 with good success at 29.9mg. Other Meds and Supplement .01 mg of biodentical Estridol and 100 mg progesterone (bioidentical) Vita C, Vita D, Omegas, Probitoics, Multiple Vitamin, Melatonin 2.5 mg. In answer to your questions about liquid.  There are some people on this site that can get to the small doses needed to taper using pills and cutting and weighing.  I personally don't know how they do it, but they do.  I went to liquid and found it much easier. Once I got the bottle of liquid it said either in the typed prescription label, or on the bottle, that so much of the ml was equal to a certain amount in mg.  I then took my bottle to my doctor to make sure I was reading it correctly.  Once I knew how much was equal to what, then then I started doing the math every time I decreased. If you decide to stay on the pills then perhaps you might want to do some additional searching to see if you can find someone that has gone down using pills or is currently doing so. If you go with liquid you might want to consider having one or two people check you math. I did that because I have seen a few people on this site get mixed up and end in a bad situation. This site advises when going on liquid we slowly transition over to it. That means the first week or so you get you dose as follows: 75% from pill and 25% from liquid, then it is 50% and 50%, then 25% pill and 75% liquid, finally 100% liquid.  No tapering during this time.  I did not do this and I did feel a little bit of a set-back. Congratulations on getting to 23.3, that is fantastic. I hope your taper goes smoothly.

Feb. 2016 to June 2016  - Was on 150mg Zoloft.  Put on Gabapentin at 900mg a day in 2016 due to antidepressant withdrawal. 

Quit Zoloft (Sertraline) June  2016,  reinstated 50mg of Zoloft July 2016.  From July 2016  to October 2016 went from 50 mg down 2.3 mg. I up-dosed in November 2016 to 12.5 mg. Held there until January 2017 when I started a much slower taper.

Thank you for your response.  I have held off on the liquid for a couple of reasons.  The main reason is dealing with the dosing daily. With the pill cutting I cut all pills the same amount for two weeks at a time and place in pill boxes.  Then really forget about it.  When I withdrew from Benzos I had to mix with milk and prepare daily and it made me mentally very stressed.  The Cutting right now does not seem to be an issue at 23.3 I use 50 mg tablets.  I will tell you that I made a switch to 100 mg tablets to 50 mg tabs and miscalculated and that got me in trouble back in March.  I see you are totally off what a wonderful feeling that must be , Ihope you are doing well.  I am afraid when I get too low it will get difficult with the tabs but I guess I will have to see and play it by ear.  Again congratulations.

1991- Put on Prozac for menstrual problems 1991-2000: On Zoloft most of this time , 100mg . Max .2002 - Miscarriage . Proceeded to go off medication due to this. Got pregnant 2nd and was off medication for first trimester. Servere withdrawl problems went back on after first trimester. .2003-2010 Continued on Zoloft, Strattera 2010-2014 Lamictal Added. 2016 - Frontal Lobe Brain Tumor Discovered 2017 - Brain Surgery - stopped any attempts to taper. 2019 - 6 month polydrugged episode that included Lorazapeam,Rexulti,Pristiq,Gabapentin,Cymbalta,Ensam (after off antidepressants), Latuda, Abilify and Prozac. End of 2019 in October ended up in hospital with Akathesia and a slew of other symptoms. 2020-2021 Successfully tapered off: Lamictal, and all other meds except Lorazapam . Did a taper for 18 months off Lorazapam - Milk taper. 2021-current :Been tapering off Zoloft since February 2021 with good success at 29.9mg. Other Meds and Supplement .01 mg of biodentical Estridol and 100 mg progesterone (bioidentical) Vita C, Vita D, Omegas, Probitoics, Multiple Vitamin, Melatonin 2.5 mg. @Gigi66 Yes, the liquid was a lot more work, I agree with you on that.  I am sure you will figure out the right course of action for yourself when you get down to the lower numbers.  Congrats again.

Feb. 2016 to June 2016  - Was on 150mg Zoloft.  Put on Gabapentin at 900mg a day in 2016 due to antidepressant withdrawal. 

Quit Zoloft (Sertraline) June  2016,  reinstated 50mg of Zoloft July 2016.  From July 2016  to October 2016 went from 50 mg down 2.3 mg. I up-dosed in November 2016 to 12.5 mg. Held there until January 2017 when I started a much slower taper.
