Tips for tapering Zoloft (sertraline)

Thank you so much for this topic and helpful answers! We tried to make a solution with water today, and the taste was not at all as bad as expected. It was however, difficult to disolve it. I understand this is not necessary if you shake it hard before taking out the rigth dose. Do you however, know if it disolves better in milk? I also still wonder if the transition to water solution is smoother than the transition to mixture?

Hi Haap,. I am on the point of changing from Prozac Liquid to mixing my own with water.

When I started liquid prozac after capsules I had a heavy withdrawal response though I did not cut the dose.

For this reason I am anxious about switching to using my own mix.

Will be interested to hear how it goes.

On 20 mg of Prozac for about ten years. Sept 2012 started reducing 10% a drop using gram scale, with average of one month holds.

When I'd reached the half way mark, taking 10 mg  powder out of the 20 mg capsules, I switched over to 10 mg capsules and cutting

down from those. Withdrawals got harder the lower I dropped.  May 2013 changed to 5% drops, holding until all withdrawal symptoms gone.

January 2015 changed to liquid prozac (concentration of 20MG per 5 mL) using a 1mL oral syringe.

Current dose of fluoxetine solution equivalent 3.4 mg. Any effort to drop below this has been disastrous so for the time being I'm staying at this level.
Adding 200 mg Tryptophan and 200 GABA a day has helped with anxiety.
Also take 1,300mg Omega- 3,  875mg  Magnesium, 1800mg Curcumin, 1000mg Vit C, 5000 Vit D.

I wish we could get liquid Zoloft in Canada.  I could only break my capsule and guess at half doses of 12.5 mg.  I guess that's why my tapering has not gone well.  I feel that Canada big Pharma has purposely made Zoloft only available in capsules and smallest 25 mg so that we have to stay on it forever!

Zoloft for nearly 20 years for panic attacks

tapering for approx. last 6 months.  3rd time attempting

withdrawl symptoms feel like a nervous breakdown

depression now - not prior to Zoloft

I just asked this on my personal topic but feel it belongs here. What strategies have people used as far as when to drop to zero with sertraline? I'm still at 25mg but would like to start planning for the end game. Thanks for any thoughts.

KEY: Drug @CurrentDoseMaxDose (time span). Details.
Prozac @4040 (2020-present). Replaced 50mg Zoloft with 10, then 20, then 40mg Prozac starting Sep 2020. I was not doing well at 25mg Zoloft, 150mg Wellbutrin XL. So, to try something new, I switched to Prozac. I am definitely more stable on this dose. Nevertheless, issues with insomnia, and worries about bone strength, make me wish to try a taper again sometime.

Zoloft @0250 (1998-2020). 250mg most of the time. Starting Spring of 2009, tapered by 25mg steps down to 25mg/day by Oct 2010. Emotional crisis prompted return to 200mg by end of 2010. Reduced to 150mg in Sep 2012. Dropped to 125mg 1 April 2013. Dropped to 100mg around 1 June 2013. Dropped to 90mg 5 Dec 2014. @80mg 5 Jan 2015. @70mg 5 Feb 2015. @60mg 5 Mar 2015. @50mg 5 Apr 2015. @45mg 7 Sep 2015. @40mg 5 Oct 2015. @35mg 1 Nov 2015. @30mg 1 Dec 2015. @25mg 29 Dec 2015. @50mg Nov or Dec 2019. Transitioned to Prozac Sep 2020.
Wellbutrin XL @150300 (2004-2020). Started at 150mg, quickly ramped up to 300mg. Dropped to 150mg in summer of 2010. Emotional crisis prompted return to 300mg by end of 2010. Dropped to 150mg 9 Apr 2014. Tried going back to 300 1 Apr 2020; feared it was harming my memory and so cold-turkeyed off 1 May 2020.
Klonopin @01 (1998-2005).
Paxil (1997-1998).

Brain, it's quite a few months before you need to worry about going to zero. You need to see how you feel when you're under 5mg to make decisions about this.

Just a quick note. I was unable to locate a liquid version of Zoloft here, or 25mg tablets, for that matter. My doctor suggested that we use a compounding pharmacy. That has been a really good - though a little more expensive - solution for me.

2005 St John's Wort / 2006-2012 Lexapro 20mg, 2 failed attempts to stop, tapered over 4.5 months in early 2012

January 2013 started Sertraline, over time worked up to 100mg

July 2014 Sertraline dropped from 100mg to 75mg, held for six months, slower tapering until 2019 22 Dec 3.2mg

2020 Sertraline 19 Jan 3.1mg, 26 Jan 3.0mg; 1 Mar 2.9, 7 Mar 2.8, May (some drops here) 24 May 2.5, May 29 2.4, June 21 2.3, June 28 2.2mg,  July 4 2.1mg, July 24 (or maybe a bit before) 2mg, early Nov switched to home made suspension; 29 Nov 1.8mg; approx 25 Dec 1.6mg)

2021 Some time in about Jan/Feb realised probably on more like 1.8mg and poss mixing error in making suspension; doses after 10 Feb accurate; 10 Feb 1.6mg; 7 Mar 1.4, continued monthly

10% drops until 1mg, then dropped 0.1mg monthly.

May 2022,0.1mg, now dropping 0.01mg per week

Oh, I should have said - I'm in Australia. I've looked on the medicines website for Australia and only found scored tablets of 50mg and 100mg. I can't think of anywhere else to look but haven't yet asked my pharmacist. Certainly my doctor thought that once we were off doses that were easily achievable with whole or half tablets of those doses, it was best to go to a compounding pharmacy.

Sounds like a good plan, Bubbles. 

 

2020: After 18+ years (entire adult life) on Paxil, a dangerous doctor-led "taper" in 2015, and four years tapering off the last 1 mg thanks to SA and the Brassmonkey slide, 

I AM COMPLETELY FREE OF PAXIL! ! ! ! ! ! ! ! Forever.

 

2021: Began conservative, proper, CNS-respecting taper of Zoloft, led by the only expert on me -- me. Making own liquid. 5-10% plus holds.

2022: Holding on Zoloft for now. Current dose 47 mg. Hanging in, hanging on. Severe protracted PAWS, windows and waves. While I may not be doing "a lot" by outside standards, things are graaaaadually getting better

 

Yoga (gentle to medium); walks; daily breath practice; nutrition, fruits/veg; nature; water; EastEnders (lol); practicing self-compassion, self-care; boundaries; connection; allowing feelings; t r u s t ing that I, too, will heal. (--> may need to be reminded of this.)

Bubbles , read this topic from the beginning. You can make your own liquid from 50mg tablets.

1987-1997 pertofran , prothiaden , Prozac 1997-2002 Zoloft 2002-2004 effexor 2004-2010 Lexapro 40mg

2010-2012Cymbalta 120mg

Sept. 2012 -decreased 90mg in 6months. Care taken over by Dr Lucire in March 2013 , decreased last 30mg at 2mg per week over 3 months. July 21 , 2013- last dose of Cymbalta

Protracted withdrawal syndrome kicked in badly Jan.2014 Unrelenting akathisia until May 2014. Voluntary hosp. admission. Cocktail of Seroquel, Ativan and mirtazapine and I was well enough to go home after 14 days. Stopped all hosp. meds in next few months.

July 2014 felt v.depressed - couldn't stop crying. Started pristiq 50mg. Felt improvement within days and continued to improve, so stayed on 50mg for 8 months.

Began taper 28 Feb. 2015. Pristiq 50mg down to 45mg. Had one month of w/d symptoms. Started CES therapy in March. No w/d symptoms down to 30mg.

October 2015 , taking 25mg Pristiq. Capsules compounded with slow-release additive.

March 2016 , 21mg

Oh yes, all good. :) The doctor suggested this, and it's working for me - at least partly because having x weeks of capsules encourages me not to rush! You do make me think of something though - when I get right down to the pointy end, getting a solution (either made by me, or the pharmacy) might be a good idea.

Old thread but reading through brought up a question. Could liquid doses of Zoloft be inserted into empty capsules and then swallowed? It could avoid the whole "drug sticking to the side of the glass" problem. Could prevent burning or foul taste problems as well. Just food for thought.

April 26th - 36.5>32.8mg Z.

Feb 4th - 40.5>36.5mg Zoloft.

Jan 5th - 45>40.5mg Zoloft.

Dec 6th - 50>45mg Zoloft.

Nov 1st - 53>50.0mg Zoloft. Sep 22/17 - 50.0>53.0mg Zoloft. Sep 18/17 - 59.0>50.0mg Zoloft.

Aug 7/17 - 65.6>59.0mg  July 18/17 - 72.9>65.6mg. June 18/17 - 81>72.9mg 

May 28/17 - 90>81mg.  May 8/17:  Started my taper. 100>90mg

1995 to May 8/17:  100mg Zoloft/day.  Working well but suspecting some signs of Tolerance this past year.

4/5/17:  Started Testosterone Replacement Therapy via T pellet insertion.  Diagnosed with Secondary Hypogonadism.

Supplements:  1000mg fish oil, 10,000iu Vit D3 with K2, 400mg Magnesium.

I’ve been given liquid sertraline to help me taper off more easily. I started taking it 4 days ago ,I fill the syringe and put the liquid straight into my mouth. The thing is I’ve been getting really bad heartburn. I’ve read on various sites that you should mix liquid sertraline in water but it doesn’t say to mix it with water anywhere in the instructions does anyone else take this straight instead of mixing with another liquid. Thanks for any info

Started sertraline in 2016 for panic and anxiety 

tapering by 5%.

changed from pill to liquid form in April 2018 so have just started a 5% taper

HOW TO USE: Read the Medication Guide provided by your pharmacist before you start using sertraline and each time you get a refill. If you have any questions, ask your doctor or pharmacist.This medication must be mixed with another liquid before use. Just before taking, carefully measure the dose using the medicine dropper provided. Do not use a household spoon because you may not get the correct dose. Mix the dose with a half cup (4 ounces/120 milliliters) of water, ginger ale, lemon-lime soda, lemonade, or orange juice. Drink all of the mixture immediately. Do not use other liquids to mix this drug. The mixture may appear cloudy, which is normal and harmless. Do not prepare a supply in advance.Take this medication by mouth as directed by your doctor, usually once daily either in the morning or evening. 

 

 As you are getting heartburn taking it neat you should try diluting it.

1993 - 2002 in and out of hospital. every type of drug + ECT. Staring with seroxat

2002  effexor. 

Tapered  March 2012 to March 2013, ending with 5 beads.

Withdrawal April 2013 . Reinstated 5 beads reduced to 4 beads May 2013

Restarted taper  Nov 2013  

OFF EFFEXOR Feb 2015    :D 

Tapered atenolol and omeprazole Dec 2013 - May 2014

 

Tapering tramadol, Feb 2015 100mg , March 2015 50mg  

 July 2017 30mg.  May 15 2018 25mg

Taking fish oil, magnesium, B12, folic acid, bilberry eyebright for eye pressure.

Lesson learned, slow down taper at lower doses. Taper no more than 10% of CURRENT dose if possible

I believe that there is a liquid sertraline available in the US. If you've ever had it - what sort of expiry date does it have? I've googled but can't find this information, and as it isn't available in my country, I can't ask a pharmacy here.

Medications: Gabapentin, Prednisone 1.5mg a day, Cortisol Inhaler daily. 

HISTORY FOR ZOLOFT TAPER

Feb. 2016 to June 2016  - Was on 150mg Zoloft.  Put on Gabapentin at 900mg a day in 2016 due to antidepressant withdrawal. 

Quit Zoloft (Sertraline) June  2016,  reinstated 50mg of Zoloft July 2016.  From July 2016  to October 2016 went from 50 mg down 2.3 mg. I up-dosed in November 2016 to 12.5 mg. Held there until January 2017 when I started a much slower taper.

Bubbles, it was not compounded. It was liquid Zoloft purchased from the drug store with prescription and came from manufacture. It is available in US. Some doctor in US don’t know it’s available, and like you said Bubbkes, not available in all countries.

April 29, 2022 900 mg to 800 mg (11%), May 29, 2022 800 to 700 mg (12.5%), June 20, 2022 700 to 650mg (8%), July 20, 2022 650 to 575 (12%), August 20,  575 to 500 (13%),  Sept 20, 2020 500 to 475mg (5%) Nov 7, 2022 475 to 425 (11%), Nov 21, 2022 500mg

Medications: Gabapentin, Prednisone 1.5mg a day, Cortisol Inhaler daily. 

HISTORY FOR ZOLOFT TAPER

Feb. 2016 to June 2016  - Was on 150mg Zoloft.  Put on Gabapentin at 900mg a day in 2016 due to antidepressant withdrawal. 

Quit Zoloft (Sertraline) June  2016,  reinstated 50mg of Zoloft July 2016.  From July 2016  to October 2016 went from 50 mg down 2.3 mg. I up-dosed in November 2016 to 12.5 mg. Held there until January 2017 when I started a much slower taper.

Thanks. Much appreciated. I've looked it up and it looks like 1ml = 20mg. So to be on a "normal" dose of 100mg, you'd use 5ml. It says that there is a graduated dropper for dosing, but I can't find any information on it. Is the graduated dropper that comes with it enough for the fine dosing that I'm likely to be doing? My calculations are looking like some doses would want 0.36ml and similar (well, I've rounded that, it said 0.3645). I'm about to phone my pharmacist and see if he can import it. Probably not, but no harm in asking. Thanks. (Hopefully this is my last question!)

Hi everyone

 

Another question - I've looked, but can't see an answer.

 

For those of you who mix up your own liquid zoloft/sertraline, how long does it keep? I've got 50mg tablets, so - at present, I'd be taking a bit under a fifth of that, so I'd want a liquid to last a week for now. Ideally, as I get lower, I'd hope it would last longer so as not to waste too many tablets. I guess once I'm at 1mg, I'm happy to toss the leftovers after a month. :) Any thoughts? Or is this answered somewhere else?

 

Compounding is getting expensive, and I'm at a bit of a nitty gritty stage now. I've been content to go down in whole milligram drops, but now I'd like a finer taper, if I can achieve it easily.

I see in the liquid thread something to the effect that they'll keep for a few days. Guess I'm hoping that someone knows that zoloft lasts longer, or I'll have to just call it, say 3 days, to be safe.

I saw that Altostrata posted the following regarding suspensions in general (not specific to sertraline) in 2016 in this thread: how to make a liquid from a tablet

Went on "exit strategy" at 0.084 mg Nov 2020 for 8 weeks, 0.042 for 4 weeks, 0.028 for 2 weeks starting 2/25/21, then 0.014 for 2 weeks, then .007 for a while, Done May 4, 2021. Unfortunately due to circumstances I restarted at 25 mg January 2022. New taper began January 2023.

This depends on the drug and the storage conditions, none of which we know.

 

If you think the strength of your home-made suspension deteriorates, discard it sooner.

Just trying to put my mind at ease. 

 

Is putting a Zoloft tablet in an empty capsule alright?  They taste badly.  Is it alright if the pieces are broken before putting them into the capsule?  I'm not doing that yet but thought I might down the road. 

 

There isn't anything specifically within this thread and various medication websites state not to crush, cut, or chew them.  It doesn't say why though.  Thanks in advance 😀 

Cymbalta:  Currently 48.5 mg.  Previously 60 mg, 2011-Oct 2020.  

Taper Oct 11-13 (three day reduction of 30 mg relieved many Serotonin Toxicity symptoms but WD became too harsh). Oct 14- 24 (eleven day reduction of 10%, 54 mg).  Oct 25  (began second reduction of 10%, 48.5 mg; holding to stabilize and work on schedule).  

Zoloft  100 mg. 1993-2000 Stopped CT for pregnancy twice, then it pooped out.  Reinstated 2011-2020.    

Taper attempt:  Oct 17-24 (seven days at 87.5 mg was discontinued; reinstated to 100 mg). 

 

Buprenorphine: 8 mg (4 mg 2x day @ 4pm and 1am)  Atenolol: 25 mg (max 2x day; prn).  Xanax: .125 mg (max 3x day; prn; average 2x).  HRT: Transdermal weekly patch.  Melatonin OTC:  2.5 mg, sleep.  Magnesium Citrate OTC: 125 mg, dinner.

Oct-Nov 2020:  Discontinued Simvastatin 20 mg, Magnesium Glycinate, Vitamin D, and Chia Seed Extract. Growing sensitivities.

The scoring on the tablets indicates they can be split. Cut them up with a pill cutter, available at any drug store.
