Hibari: tapering Remeron and want to start tapering Lamictal

Hibari

Recommended Posts

Hibari,

Sounds like a hard day.

I was just reading your signature and noticed all the changes with the Remeron. You've been up and down, even though it's in small amounts...and switched to liquid, plus altered your hormones, and have done a number of different supplements. I'm not an expert on this, but seems like all of that could contribute to more instability.

But, I'd also say I've found it impossible to distinguish at times what is grief and what is WD. I had a lot of anger and rage, plus sadness, and a whole range of other feelings after my wife died. I was on a board with other widows, so know that we were all going through very similar stuff. And it would often strike out of the blue the way WD symptoms do. I'd latch onto something or other from the past or what someone said to me, or the way the person in front of me was driving...and off I'd go, sometimes for hours at a time. My thoughts were crazy at times too--the same as people describe on here.

I started tapering about a year and a half after she died...then, I couldn't tell what was wd and what part was grief. I think they were likely mixed together....and maybe one more than another at times. As I said, anniversary or holidays are especially heightened..and you said this was the time your mother went into hospice care. I'd suspect that is fueling things as well.

Have you been able to talk with someone about your grief? I'm thinking that most hospices provide counseling for family members after a death...and there is often group support as well. It could be helpful with that piece of things.

You probably already know this, but when someone dies, all the unsaid things come up...all the ways we've given ourselves away..regrets, everything really. It's not pretty, but it's part of the process of healing. It's very normal to be angry, even under better circumstances than you've described with your mother. I was furious with my wife, the doctors, with family, friends, and everyone who was going on with life, while I was trapped in this state of limbo and so much pain. I could barely tolerate being around people, maybe even worse than in wd. Everything grated on me.

In a way, what matters most is how we lean into whatever is coming up...and not so much the source of it. We can make ourselves crazy and unstable by chasing the symptoms or pushing them away or trying to figure it all out. What things can help you meet what's arising? Or distract from it at times, to give yourself a break. Some things we need to find our way through, however unpleasant...but it helps having some ways of working with what comes up.

Remeron for depression. Started at 7.5 mg. in 2005. Gradual increases over 8 years, up to 45 mg. in 2012.Began tapering in June 2013. Went from 45 to 30 mg in the first 3-4 months. Held for a couple of months.Started tapering by 3.75 mg every month or 2, with some longer holding periods. Eventually went down to 3.75 mg. about April 2014. Stopped taking Remeron August 2014. Developed issues with histamine a week after stopping--symptoms reduced through diet and a few supplements. Currently having issues with a few foods. Most of the histamine intolerance has resolved or is at least, in remission.

Thank you so much.  Your words really clarified a lot of what I am feeling.

I have gone up and down with my Remeron but am holding now at 28mg-liquid.   My hormones I have to pay attention to because I will start to get a series of hot flashes that disappear once I change my Estradial patch.   The supplement L-Theanine has done wonders for my anxiety.  I also am on Armour Thyroid but have been for years.  I am not going to adjust the hormones till after I come off of the Remeron.

I was just speaking to a friend tonight about the grief that is coming up around the care taking of my mom.  Yes, there is now the time and space for it to emerge. I just need to let it out when it comes.

I go to meetings and talk out a lot of my feelings around this, which is helpful.  I have stopped therapy two months ago because it didn't feel right to be there anymore.  I don't feel the lack of it even with all the deep feelings that are coming up.  Just don't want to do that kind of work anymore.

You're right-if I try to stop figuring out what is coming from where it will keep me from being so anxious.

Thanks again for listening.

Sorry Hibari...you already said about stopping therapy and I'd forgotten that. It sounds like you've found exactly what you need for this time and that's great. Support can come in many different forms. I found grief counseling with hospice to be much different than therapy, partly because the woman I saw there was exceptional..and it's also a different approach to things.

People are often shocked by how intense grief can be...and feel like they are going insane. It's a tough road to travel, but something every single one of us will go through. It's just unfortunate I think that we are so ill-prepared and that there is so little cultural support for the process. It can also be a great opportunity for change and healing, given the right conditions.

It's true, grief is not well understood or supported.

Today I cried multiple times and the deep feelings that were pouring out of me were all grief and anger.  There was also deep crying around finding me again after all this time.  Crying that had to do with acknowledgement of who I have always been.  A pouring out of compassion for myself.

I was in an Alzheimer support group for the last 2 years of taking care of my mom.   It was a good group and helped me feel less crazy around the feelings that came up.

I realized that as I write this, that I miss my mom, the mom I took care of for all those years.  We had some sweet moments together and even though it was hard to present all the time.  I'm glad I had those moments and I know she loved me.  For right now, the grief and WD are all mixed together.   As you mentioned in one of your responses, I don't need to figure out what comes from where-it just is.

Feeling pretty good today after about 3 days of sorrow and anger.  Besides the feelings that are coming up, I realized I was experiencing the Remeron pattern of withdraw for me.  The physical hits first then my mood is sorta flat and then during the second or third week after the cut, I feel very down and then cry uncontrollable for two days, then a bit sad and then better.  The switch to liquid gave me an unexpected cut though I don't know by how much.

I still have a flat feeling, not so inspired or full of optimism but it's a relief to feel happier.   I am looking forward to finding my joy again and living life.

You might find that smaller cuts have less symptoms and are easier to accommodate.  See the thread on micro-tapering.

Lots of people here do 5% decreases but more often , every 2 or 3 weeks.

Apologies for thinking you'd been on the meds. 10 years (earlier post).   Note to self: read things more carefully.

Oh , and you can call me Silver Star anytime , lol.

Me again, got stuck in my writing for some reason.

My question is: I now taking a suspension of Mirtazapine , 28mg pill equals 7ML liquid.  The syringe shows that there are 5 lines between 7ML and 6ML.

Should I go line by line for my micro cuts?

Math isn't my thing , but I think this is correct.

10% decrease of 7ml would be 0.7ml , which would bring the dose down to 6.3mg.

5% decrease of 7ml would be 0.35% , which would bring the dose down to 6.65mg.

Each line on the syringe between 6 and 7mg is 0.2mg.   Decreasing by increments of 0.2mg should be fine for now ,

but as the dose becomes smaller , 0.2 is a larger percentage.  For example , when you're on 1mg , 0.2 will be a 20% decrease.

Thank you.

Tough two days.  Very very weepy and tons of hot flashes.   I am on bio identical hormones and also have been on an antibiotic for a possible tick bite for 11 days, 100mgs twice a day.

I couldn't understand why I was having hot flashes again and then I read that antibiotics can affect hormones-especially estrogen.  The information is across the board-some experts say no while regular people seem to say yes.  In addition to that, and I can only write this here, I now have a fever blister on my lower lip.  I have felt very run down and tired the past two days and I think I do have a bug.

I am not changing anything till after I am off the antibiotics but I am despondent tonight.  So scared I won't make it through this process because my nervous system has been compromised. I am able to work some but not feeling so determined in my goals in life.   I could cry about what I have been through in the past 2 1/2 years and the thought of it continuing like this is so disheartening.

Sorry things are so rough right now. It can be very disheartening at times. When you're in the midst of things, it's easy to think it will stay the way it is...but there's also the possibility you could stabilize and things could be a lot easier than they are now. It's more complicated when you're dealing with multiple things, in addition to WD.

I think a lot of people suffer from temperature disregulation..if you search on here, I believe there's a thread. I've had hot flashes too, and hadn't had them for years. The whole endocrine system is affected by wd, so it's not surprising that this happens. Are you having them at particular times?

While I love my kitty to come visit under the blanket in the AM..as soon as she does, I'm hit with very high temperatures...I had a lot more trouble handling summer heat over the past 2 years too...so far, seems better this year.

Thank you so for your support. I am better and have been since Friday morning. I worked on a project all day Thursday even though I woke up feeling shaky.

I think working all day drained all my anxiety off.

Anyway I woke on Friday and felt better and the same today. I think I have stabilized on the liquid suspension I am using.

To answer your question I usually wake up and have a hot flash. I think I am sweating during the night.

I have about 4 days left of my antibiotics. Then I will assess my hormone levels again and take a tiny micro taper. I want to try the micro taper route now that I know my pattern.

I really appreciate your kind support and will take another deep breath and go forward.

I also am experimenting with a sleep mask to have more control over the way my cortisol rises in the morning. I will report back what I experience.

You might want to think about giving your brain and body a rest from adjusting for another week or two.

Let yourself stabilize a bit - you deserve more than a few days symptom-free before doing it all again.

Protracted withdrawal syndrome kicked in badly Jan.2014 Unrelenting akathisia until May 2014. Voluntary hosp. admission. Cocktail of Seroquel, Ativan and mirtazapine and I was well enough to go home after 14 days. Stopped all hosp. meds in next few months.

July 2014 felt v.depressed - couldn't stop crying. Started pristiq 50mg. Felt improvement within days and continued to improve, so stayed on 50mg for 8 months.

I have shifted over to Leahy's posts regarding Remeron withdrawal and have been posting there but wanted to post here today.   I am feeling very emotional today-I call this my monthly Remeron wd meltdown.

I cried last night felt better but woke up with sweating and feelings of despair.  All day long I have felt this meltdown getting ready to happen.  I know I'm not depressed because I can see that today it's a sunny day and knowing that does not make me feel pain the way it used to when I was depressed.  I just feel jaggedy and sad, angry and mournful.

I will reach out to a friend of mine who holds the space for me while I cry and rant.    I need to do that.

Having a tough day today.  Woke up sweating, depressed and scared.   It's hard to figure out what is WD and what are menopausal symptoms, which I have had for 3+ years.   It is infuriating to me that my body is not in balance and that I am not waking up with an optimistic baseline.  Even when I meditate I feel despairing and anxious.  Take my L-Theanine for anxiety and am going to make sure I have a steady enough amount in my system.

Remeron makes me so sedated and I wake up in a stupor in the morning and it takes about 3 hours to clear my head.

Cried my eyes out last night and even with that woke up sad.   Believe or not, I am hoping this is WD.

Hi Hibari,

I wake up sweating also and menopause was long time ago. it's weird to wake up like that - sweating and a very high intensity headache. Today once again I feel hungover. The thing I look forward to is the day being over so I can try again to get some good sleep. I feel like I am wasting so many good days feeling terrible all day.

I wonder... Do you get the chance to get out in the sun for a gentle walk? The sun can give you a boost even if it's for two minutes. 20 minutes would be better but 2 is better than nothing. That's how I started (with two minutes) to make the "get out in the sun effort" I think that might hep a bit with the crying and sadness.

I remember crying while talking to my son one day and he said "Ma. What do you wanna be crying for - on a Saturday? Stop."  I started to laugh then. I said "Well, I didn't PLAN to start crying!" Kinda bewildered like. But I did stop crying and moved on.

There's a range of emotion right there, huh?

I think I might feel sad if I felt physically better or happy. I have no idea. I can't get past the headache and nausea and tiredness. I just wanted to share some of this bumpy ride with you. I bought Alka seltzer Gold yesterday as you suggested. Thanks for that tip.

Thank you for sharing that you wake up sweating and that menopause is behind you.   At least I feel less crazy that my whole system is out of whack, well I do feel my system is not in balance.  I am also going to find a gynecologist to test my hormones again.   That hungover feeling of Remeron was the first sign to me that I was over medicated.  After about 10 months of being in a cocoon of medication, I found myself really wanting to shed that feeling-hence me coming to this website.

I also have double checked that my other medication, Lamictal is affected by the Estradial patch that I am on.  It apparently impacts it's effectiveness.  I am not going to even touch that right now and wait till I am off the Remeron.  It's funny, my psychiatrist said that it was the reverse, that Lamictal affects the effectiveness of the patch.  Who knows at this point, I just have to take this one step at a time.

I do plan to get out with our dog a bit later today and get into the sunshine.

I too feel badly that my days are being wasted.  I am able to do some things and work some, which is a relief but I feel that life is passing me by.

I understand how those headaches can be overwhelming. I said to my former therapist that I knew it was withdrawal and not depression returning because when I was depressed I did not get a headache or nausea.  I do know that they ease up when the brain and body stabilizes.

Glad to share this bumpy ride with someone who is on the same part of the journey.

Hi Hibari

Just reading your signature and some of your posts. I'd like to pose two ideas. 1). That you hold off making any changes for a while. Give you and your nervous system a holiday. I have long holds periodically and it helps to really stabalise, particularly when your system is already up and down with menopause. I know you want off but the idea is to come off well - longer holds contribute to that. You may even find with a stable base you can go a little quicker.

I also wonder whether splitting your dose up will help with the hangover. Or changing the time when you take it?

Your thoughts?

I offer an opinion based on my own experiences, reading and discussion with others. On Effexor for 2 months at the start of 2005. Had extreme insomnia as an adverse reaction. Changed to mirtazapine. Have been trying to get off since mid 2008 with numerous failures including CTs and slow (but not slow enough tapers)Have slow tapered at 10 per cent or less for years. I have liquid mirtazapine made at a compounding chemist.

Was on 1.6 ml as at 19 March 2014.

Dropped to 1.5 ml 7 June 2014. Dropped to 1.4 in about September.

Dropped to 1.3 on 20 December 2014. Dropped to 1.2 in mid Jan 2015.

Dropped to 1 ml in late Feb 2015. I think my old medication had run out of puff so I tried 1ml when I got the new stuff and it seems to be going ok. Sleep has been good over the last week (as of 13/3/15).

Dropped to 1/2 ml 14/11/15 Fatigue still there as are memory and cognition problems. Sleep is patchy but liveable compared to what it has been in the past.

I think I mentioned before that I hadn't been having hot flashes or night sweats for a long time before WD. The exceptions to that were in spring and fall, where it seemed my body had more trouble with regulating temperature. I'm not totally convinced that the excessive heat I have at times now is the same thing. For one thing, I can now be cold and hot at the same time. I know that doesn't make sense, but I do feel both chilled and overheated sometimes in the same moment. It also has a different quality than it did before, though I'd be hard pressed to explain the difference. I have a sense though, that the problem lies more with the nervous system and less with female hormones. But for me, I'd come through menopause quite a few years ago.

In the fall and early winter, I simply could not get warm. I'd crawl into bed after lunch, because I was so cold I didn't know what to do. I'd have a heating pad, several layers of clothes, a duvet, extra blanket and 2 cats....and it would still take me awhile to heat up. Sometimes even a hot bath did nothing to warm me up. This is part of why I see the hot flashes as an aspect of nervous system dysregulation..and perhaps is affected more by change of season than is normal too.

Dalsaan, I have never heard or just haven't readt yet that coming from a more stable base can make things go quicker.  As you have probably seen in my posts, it is the toughest think for me to do.  But you saying that really helps me.  You are right, if I am coming from a shaky stabilization, the next cut will be harder.  If I know that holding will help move this process along, it will keep me there.  I know you are not guaranteeing this but I like the perspective.

I do feel slightly better today, a sign that maybe I am moving towards stabilizing at 27mgs.

I have been taking my Remeron a little later than I normally do.  Sometimes I do that when I have to walk the dog at around 11.  But, I think I still take it earlier, which I used to do in the winter and was okay, even if I was a little groggy.

Free Spirit I too could not get warm this winter and even now in the summer, when it's super hot out, I sometimes have a light blanket over my knees, well our apartment is on the cool side to keep our dog comfortable.   Yesterday, I was looking at withdrawal symptoms on another webpage that simply lists withdrawal symptoms for Remeron.  It was good to see that crying and sweating were some of the most common symptoms.    I felt less crazy and hopeless.

Hi Hibari , I've had seasons of being almost permanently cold over the years, including summers. Even now , I sometimes get heat waves , interspersed with freezingness (I fondly refer to this as "the hots and colds").

As far as I've seen , menopause only talks about the hots , so if you have both it's temperature disregulation and that's from the drugs.

You're not crazy , and you'll recover from this , so don't let yourself get too hopeless. I think Dalsaan's suggestion of a good long hold is a good one - there's no hurry here , the important thing is to look after yourself as best as you can.

Thank you Fresh.  I have been feeling better for the past 2 days.  I am in a window right now, which follows my pattern of making a cut, feeling the physical reaction, an ok week then 2-3 days of uncontrollable crying and then emerging into stabilization.

The temperature thing is wild.  It's very warm here yet i continually go out in a sweat shirt and feel fine. I think there is also something about being protected by the weight of the sweat shirt at a time when I feel the most vulnerable.

Thank you for your support and I hope you are doing all right yourself.

Still in a window today-want to document it so I have something to refer back to.   Today, it's an okay day-not feeling particularly up but certainly not down.   I've posted before that in addition to the WD process, I believe that the 200mgs of Lamicatal is muting my affect and normal personality.  Not so much that I don't have feelings or energy, but generally a feeling of being muffled.  It could be the Remeron as well, which has a sedating effect.  I want to do my next drop in mid July and then one probably towards the end of August.  I will gauge how I am feeling but I want to do this while it's summer time and there is plenty of light.

So exhausted today and have been for the past 2 days.  I feel very foggy and spacey and it feels like I am floating in a bubble.  Sweating like crazy during the night.

I feel nervous, I have a work commitment tomorrow where I need to be on with my energy.  I am going to do the best that I can and then rest after that.

I have to take care darning the next few weeks, it was over doing that created depression in the first place for me.

Feeling a bit flat and sad over the past few days.  I haven't made any cuts in my medication-still holding at a liquid suspension of Mirtazapine 27Mgs.

I need to hold here for another 5 days due to work and some people visiting.   Then I will make another 10% cut and get myself through to stabilization.

Even though I am eating well, gluten, sugar, dairy and alcohol free, my weight has not budged.  I know this is a result of the Mirtazapine but as I have written above, it still affects my self-esteem.

Not a horrible day, just blah.

Hi H,

I am also eating well but my weight has not budged perhaps even gaining weight. All my "fat" clothes are tight and uncomfortable.  I honestly look 9 months pregnant as well. I am extremely - extremely bloated and embarrassed to be seen in public. Holding at 27 until my daughter goes back to San Francisco next week. Mirtazapine withdrawal is torture.

Why not try a 5% cut next time and see how you go?

With smaller cuts you can have shorter holds - see brassmonkey's thread.  You might find the smaller decrease is far less unpleasant and you recover faster.

Hi Leahy Yes, it is hell to withdraw from Mirtazapine. I am having a day where I worry I will never get off of it. I have been in a stabilized period, no physical withdrawal, but I woke up today sweating, depressed and anxious. Yesterday too my thoughts were not good, fearful and scares.

Is it normal to still feel depressed and scared even in a stable period? I am thinking this is a wave.

I have been working more and I think it has shaken my healing.

Fresh, thanks for the suggestion to look at Brassmonkey's thread I see how he reduces a tiny bit every 2 weeks to get to a 10% reduction. I have some time off at the end of this week and then a lighter work schedule over the summer so I can try that for a month.

H,

I really think feeling scared is part of the mirtazapine thing. Something about mirt being noradrenergic, I think. I know I often wake up feeling anxious, overwhelmed and scared. Once I realized that it is part of the deal, I was able to deal with those feelings better. I also wake up in a complete sweat and/or panic. I think that's a cortisol surge. Sometimes I take Seriphos.

Have you tried Seriphos? There is a thread on that here. http://survivingantidepressants.org/index.php?/topic/3782-phosphatydilserine-aka-phosphatidyl-serine-seriphos-or-cortiphos/page-2

I would like to know if I am really this fat or am I bloated? It's worse than ever. My belly is HUGE!

So because we are withdrawing, the brain no longer is getting the drug that provides it with the noradrenergic feeling?  Our brain has to re-balance and find it itself.

I woke up okay today, but as the day has gone on, feel a need to cry.  There is also the flat feeling of being drugged, the better I eat the more I feel it.

I am holding off of taking any additional supplements except for Omegas and L-Theanine (for anxiety) because I am on Lamictal as well and am not sure of the interaction.

When I have read about Remeron and weight gain, lots of people talk about the weight gain feeling puffy.  Maybe that's the bloated feeling you are feeling.

Thanks for your support Leahy.  I hope you are doing okay today.

Have made another cut after holding at 27mgs for awhile - longer than I have before. So made a slide down on my Mirt suspension 2 nights ago to 26mgs. I feel a little down and have a headache. Been reading as many recovery stories as I can.

I also, for the first time am feeling the over medication feeling from my Lamictal dose. I am a big body mind believer and I trust what I feel despite my withdrawal from Mirtazapine. I am not going to touch my Lamictal dose for now but I am going to keep noticing how I feel.

I have been having weekly acupuncture and even though it can kick up some detox effects, I find it really helpful.

Hi H,

I have a very bad headache today too. I also feel so sad and discouraged because it's a beautiful beach day as I sit in the house. All I can do is hope for the day to end so I can go to bed for the night in the hopes that I could feel somewhat "normal" tomorrow.

I do not understand the headaches. Nothing helps it. I gave up on trying the two 15 mg tablets of mirtazapine and I am back to shaving off 20 scrapes to reduce my dosage. Not very accurate, I suppose but I am desperate to be under 30 mg. Hibari, where did you say you got the liquid mirt? I am going to ask for that next visit on Thursday.

I hope you enjoy a little bit of sunshine today.

Hi Leahy

I am so sorry you have had a tough day today. I know that feeling of wanting to go to bed because I am hoping to feel better the next. Hang in there. You are in WD and I truly believe it is a hard but temporary journey. You have come so far and that says a lot.

I think the last time you posted you were shaving 15 shaves off your pill. Maybe consider hanging our at 20 shaves for a bit longer? I hated the idea of staying at 27mgs for as long as I did - about 2 weeks longer than I have been due to work and a recommendation on this thread. It did make a difference to be stable longer. Now I feel more prepared to do 2 micro cuts. The first to 26mgs and then to 25mgs on the next week.

They don't have a liquid Mirtazapine in the US but a compounding pharmacy can make a liquid suspension of Mirtazapine pills. The one I use is Miller's Compounding Pharmacy in New Jersey.(they are nice and helpful) They mail me my formula. My psychiatrist calls it in. He has called in 4ml suspension, which I had the pharmacist explain to me. They give you a syringe and you use it to measure your level. My formula starts at 7ml on the syringe and I have been working my way down line by line. I found out through this forum that switching from pills to liquid created a unexpected cut because the pills and liquid are not equal in terms of potency. so I may be lower in dosage than I think.

A long post but I want you to know you have my support. We can make it through.

Thank you, Hibari. You are correct. I was at 15 shaves and now 20 shaves. I think I feel more sickness when I don't sleep long enough. I feel much, much better today. I appreciate your responses and support so very much!!

Thanks for the pharmacy info. I'll ask about the liquid on Thursday when I have my appointment. I remember reading what you wrote when you switched about the dosage not being equal in potency.

I was reading yesterday a post where ppl wrote about their cats being given one dose of mirtazapine with bad results. I was amazed to read how these cats cried and whimpered for days. They had to wait 72 hours for the effects to subside. I thought if it makes cats cry no wonder Hibari is crying. Just remember, "This too shall pass."

Sleep well,

Leahy
