Hi ,  

*14 yrs old imipramine 1978 for a year or two (DEPRESSION)( EATING DISORDER)

*Ritalin at 25 yrs old for 20 yrs 20 to 40mgs per day (ADHD)

*Zoloft150 to 200 mg  ,Effexor 100 ? and Lexapro 10 to 20 mg * ( each separately ) over the years ,per day, for 25 years ,average dosage (DEPRESSION)

*Seroquel 1200mg .05 Ativan, 10 mg ,ambien  9 years for (INSOMNIA)

*100mgs Zyprexa 50 Lamictal AND 4 mg of  Clonopin for 18 months for (INSOMNIA)

*SEPTEMBER 29 , 2017 , Cold turkeyed off Lexapro 10 mgs , short crossover to 50 mg Zoloft , a month taper off 1200 mg   Seroquel  .05 Ativan , 4 mg Clonopin ,100 Zyprexa ,50 mg Lamictal ,10 mg Ambien

When you do your signature, could you please clarify a few things from your post?  Before you cold-turkeyed off the Lexapro 10 mg, how long had you been on it?  Are you still on Seroquel 1200 mg.?  Is your Ativan dose .05 mg or .5 mg?  Are you still on 4 mg Clonopin, 100 mg Zyprexa, 50 mg Lamictal and 10 mg Ambien?

 •    A list is easier to understand than one or multiple paragraphs. 
 •    Any drugs prior to 24 months ago can just be listed with start and stop years.
 •    Please use actual dates or approximate dates (mid-June, Late October) rather than relative time frames (last week, 3 months ago)
 •    Spell out months, e.g. "October" or "Oct."; 9/1/2016 can be interpreted as Jan. 9, 2016 or Sept. 1, 2016.
