Hibari: tapering Remeron and want to start tapering Lamictal

Hi, 

 

This may have been posted before but I am having trouble finding it.

 

I am concerned with how sleepy I am.  Yesterday, on a 3 hour car trip, I could I could not keep my eyes open, fell asleep twice.  Got to our house, I slept from 4:30-7:30, had a really hard time waking up.  Then slept all night, am tired today.

 

I am not working particularly hard, only walk for exercise and eat pretty well. 

 

I know that Remeon is sedating at the lower doses and I am hoping that is what's causing my excessive sleepiness now.  I also know that Lamictal causes fatigue during wd.

 

Can anyone share about their experience regaining energy after they were med free?  Or point me towards a thread that talks about it?   I don't want to sleep my life away.

After my final dose of zyprexa I had very bad insomnia for around 3 weeks maybe more then like a light switch I became tired all the time like I was drugged.I had bad headaches and felt dizzy all the time and could sleep just by closing my eyes.Exercise has helped with this and it slowly lifted around the three month mark

Rumeron is supposed to be very sedating at low doses ,lamictal is an anti pyshcotic isn't it ? So will also be sedating

2012 put on Citalopram and diazepam for 3 months for "depression" after filling in a 3 minute form at the doctors, had a massive reaction with panic attacks and extreme anxiety,never suffered panic attacks or anxiety before citalopram.Told to quit cold turkey which led to two hospital admissions during 2012/2013

December for 6 months Seroquel dosage adjusted up and down 50mg ,150mg ,100mg, caused severe tinnitus ,told to quit cold turkey

2013 January for 12 months Lorazapam given to me like sweets,told to quit cold turkey

2013 May Zoloft for 6 months ,told to quit cold turkey, reinstated 50mg tapered 2nd time over a month (to fast but I survived)messed up my sleep

Zyprexa April 2103 5mg until august 2014 ,dropped by doctor down to 2.5mg for one month went well but sleep was very poor for 3 weeks

End of 2015 I had to reinstate back up to 5mg due to constant insomnia that wouldnt go away Started a slow taper and found an understanding doctor who listened to me while I reduced

Lamictal is a mood stabilizer and yes sedating as well. A double whammy for me.

Keep going I can tell you from experience things eventually return to normal,the body and mind is very resilient and will overcome in time

Yes, I agree.  

 

This appears to be the only major drug interaction because I take .   It may indicate that the amount of Lamictal I have in my body is lower than I think but that doesn't bother me.

lamotrigine progesterone

Applies to: Lamictal (lamotrigine), progesterone topical

Talk to your doctoricon1.png before using lamoTRIgine together with progesterone topical. Combining these medications may reduce the blood levels and effects of lamoTRIgine. You may need a dose adjustment if you have been taking lamoTRIgine and are starting treatment with progesterone topical. Similarly, if progesterone topical is discontinued, your dosage of lamoTRIgine may need to be readjusted. Contact your doctor if your condition changes or you experience loss of seizure control during treatment with these medications. When progesterone topical is stopped or interrupted briefly, be alert to potentially increased side effectsicon1.png of lamoTRIgine such as dizziness, drowsiness, nausea, vomiting, diarrhea, blurred vision, incoordination, tremor, restlessness, irritability, depression, anxiety, and mood or behavior changes. The sweating and nausea is back today.   My recent Lamictal cut from a week ago is showing up.  I'm hoping this means it's peaking right now.  I usually have a couple of days after I cut either medication where it's worse than others.   Yuk.

Another night of profuse sweating and then waking up with a wave of nausea.   I do get scared around all this because I know that being in menopause contributed to my deep depression.   I find it hard to pick through what is causing what-my mood is okay, some anxiety-mainly around not feeling that well and some foggy thinking. 

 

Anyone else go through these periods of intense sweating and did they leave once you were off meds?

 

I may have to find another gynecologist to help with the hormones though my ultimate goal is to switch over to something natural after I am off the medication.

Hibari, your anxiety is completely understandable. Menopausal symptoms are how you ended up medicated in the first place. Of course you would have a strong negative association with sweating. Feeling nauseous in and of itself scares the hell out of me - every time. I know I should be used to nausea by now. But I am not. 

 

I wake up feeling nauseous and profusely sweating, all the time. It is surely withdrawal. There have been times when the nausea and sweating have been so bad I had to updose a bit. That usually helped in the most severe times.

 

It must be hard to come to terms with the fact that even at the low doses you are on now that you still have to suffer from withdrawal.

 

I'm hoping to reassure you that you will never suffer through menopause again the way you did before. You will get well again. And happy.

Your words are so comforting to me Leahy.  Only someone withdrawing from medication would find comfort in knowing that sweating and nausea are a normal part of tapering.  :)

 

I continue to sweat during the day as well.  I also notice that my mood starts to lift as the day goes on-have to see whether that is cortisol or if there is less medication bogging me down.

Yea. Birds of a feather flock together.  :) I take multiple showers during the day to cool off. I swear I do. I HATE sweating but it doesn't worry me. I have different triggers.

 

I'm happy to hear your mood lifts as the day goes on.

Feeling sorry for myself. 

 

I am have had a rough couple of weeks, windows and waves throughout the day as opposed to 4 days of waves and then a period of a window.

 

On top of everything I am going through, my hormones continue be unstable.  I feel I can't handle one more thing but I continue to have some night sweats, followed by some minor hot flashes during the day.

 

My mood is low and I want to cry a lot, often right before or after a hot flash.  Lots of panicky feelings and despair. 

 

I keep telling myself that there is no avoiding the symptoms of tapering and that has been true for me.  Whether I cut one drug or two, at some point I have symptoms. 

 

Not much else to say except I am in a tough period, one of those periods where I think, maybe I should stay on meds.

Ugh, it's rough alright.  How long since you've had a longer than usual hold?  For some people they make a difference.  Although - ha! - I'm trying one at the moment and I'm not noticing much improvement yet (though I believe it must be helping at some level, even if I can't feel it yet). 

 

Commiserations on the added hormonal issues - it seems so unfair, on top of w/d.

2010  Fluoxetine 20mg.  2011  Escitalopram 20mg.  2013 Tapered badly and destabilised CNS.  Effexor 150mg. 

2015 Begin using info at SurvivingAntidepressants.  Cut 10% - bad w/d 2 months, held 1 month. 

Micro-tapering: four weekly 0.4% cuts, hold 4 weeks (struggling with symptoms).

8 month hold.

2017 Micro-tapering: four weekly 1% cuts, hold 4 weeks (symptoms almost non-existent).

2020 Still micro-tapering. Just over 2/3 of the way off effexor. Minimal symptoms, - and sleeping well.

Hibari, Sometimes we need to hold for longer periods of time. Especially when we get to the lower doses. Maybe that would help you? I'm going to quote MapleLeafGirl here because I think this is good advice.

 

"i wanted to also say even though i did a 4 year taper........i would now do things a little different.....not the % i did it at...but when i got to 10mgs..i should have stayed there for 6 months or more to see if there was any withdrawal...same at 5mgs..and of course knowing what i know now i would be going down to 0.....in the past i always had DELAYED withdrawal...when went off paxil in 2007...it did  not show up until i was off for 10 months...and i ended up going back on at 11 months off....i had a weird feeling when i was tapering...that is was going just too smooth...i had no problems really at all in those 4 years.....i thought...hmmmmmm this may come back to haunt me and low and behold at 20months off...it did...so for me..that was delayed for me.....so that is what i would do different now...take longer than even 4 years....."

Yeah, I am going to hold till I stabilize.   It is such a strange feeling to know that I am so low, yet would fight tooth and nail if someone tried to force be to take more medication.

Just updating that I have been stable since last Thursday, June 9th.  I just was reading my post above and I see how much wd affects my thinking.  I know it's because my brain is not functioning at those peak wd times and does not have the capacity to be logical.

 

Holding till after a big work commitment on June 30th. 

 

Still have low grade nausea on a daily basis, which I have connected to the Lamictal wd.

Moving down to 4.8mgs of Remeron tomorrow night after I finish a huge work project.  I've been pretty stable over the past 3 weeks, still the low grade nausea and some flashes.  I may shave a tiny bit of my Lamictal as well, maybe 2%. 

 

I'm struggling with the Remeron hunger right now.  It's come back even stronger as I get lower down on the dosage.  I am trying to battle it as much as I can through chewing gum and not snacking but wow, it's pretty constant.   It's making me more determined than ever to get off.

Made the drop to 4.8mgs.  So far just the usual nausea and vulnerability.    Also feel more sweaty but it's 90 degrees out and that adds to it.

 

While I still am super unhappy that it's taken 17months so far to come down off a medication I was on for 10 months, I have no other choice.

Hibari - I second what leahy said.  You have been working at this double taper and that's been working.

 

You've cut just under 90% (89.4%) of the Remeron dose and 84.4% of the Lamictal.  These massive reductions in dosage reduce adverse  effects significantly and decrease the load on your liver. Well done!

Does your nausea go away later in the day? Do you think it's like hangover effect. When do you take your meds in the am or pm?

I'm on 1800 mg of gabapentin 3x600, .325 Klonopin(.25 in am, .125 afternoon), in the evening I take cymbalta 30mg down from 60mg, remeron 30mg and lamictal 100 mg. Nausea and anxiety are my worst symptoms. C

My nausea stays pretty constant throughout the day.  It may disappear for a bit then it comes back.   At this point it's low grade and doesn't keep me from eating.

 

I think it's mainly associated with withdrawal as it shows up more intensely right after I make a cut. 

 

I take all my meds at night so that any side effects can be slept through.  Remeron has always knocked me out from the get go and now as I am on the lower dosage, it is even more sedating.   I know that Lamictal is sedating but has never made sleepy the way that Remeron does. 

 

For any anxiety that kicks up, I am taking 100mg of L-Theanine as needed.  Sometimes not at all, sometimes 3 times a day.  When I first started cutting Remeron, my anxiety was through the roof.  I was bouncing off the walls.  My psychiatrist recommended L-Theanine, 100-200mgs 3 times a day as I needed.  It has helped me tremendously.  I found that after taking it pretty regularly, my need for it lessened and now as I said, as needed.

I am pretty stable right now.  Some hot flashes and a bit of nausea but so far the drop to 4.8 has been okay.    I added in some supplements about a week ago as well and that's the first time in a long time.  My weight has not dropped anymore but that is due to my larger portions due to Remeron hunger. 

 

Tonight I plan to shave a bit of my Lamictal dosage. 

 

And so it goes.

Made a small Lamictal cut on Tuesday night from 31.25mg to 28.125 mgs.   I was going to wait but I am getting concerned about my cognitive abilities.   Whether it's withdrawal or not, I am struggling with my short term memory and that is making me very determined to get off these meds.  

 

I was giving a presentation yesterday and I looked at someone whose name I was going to reference in the speech and could not remember her name.  This is someone I know and like very much and it unnerved me.  I was able to retrieve her name and use it later in the presentation but wow.

7/2015-11/2018 Lamictal: 200mgs-0.05 mgs Had paradoxical reaction to Lamictal wd, broke my heart to take a benzo but wasn't sleeping. 

3/28/2019 -2/5/ 2021  Clonazapam: 0.625mgs-.00115 Med Free 

July 27th, 2022**Severe Setback due to surgery/ anesthesia. 

9/7/22-10/4/22 Trazadone 50-100mgs for sleep, 10/13/22-11/13/22 Trazadone 1 mg to stabilize

10/4/22-11/20/22 Remeron 7.5mgs (for sleep doesn't work) 11/20/22 7.3 - 12/31/22 6.3

This word retrieval or name retrieval or event retrieval problem happens to me all the time. It is concerning and I realize I am only aware of it since I have been tapering. It may have happened before but I don't remember that. That figures...

 

I do notice that if I relax about it and let it go, The words lost will miraculously float back to me. But that takes a little time. sometimes a lot of time. But they do float back. Now I notice when other ppl (like my son - no meds and much younger) struggle with this phenomenon, I say to them "don't stress over it, it will float back into your head. And it does.

 

My guess is that this happens under stress to many people. I'm sure our neurons need to make better connections but I feel these connections are happening. In the meantime, be reassured your thoughts and knowledge will float back in.

 

Goodness! That had to be stressful during a speech! Kudos to you for speaking in front of a group. I am very, very proud of you. I wouldn't DARE do that at this place in time.  :)

Thank you Leahy.  Yeah, the words do float back later and that is a relief.  I love public speaking so doing it actually makes me feel more empowered. 

 

On a separate note, remember how I told you I was losing a little weight while coming down?  Well it's stopped now that I am on the low doses and I am hungry all the time.  It's that false Remeron hunger and I have to fight it like crazy.  I'm chewing lots of gum and probably drinking more ice coffee than I should.  The hunger is all for sugary carbs and if I have them, I have them but only once or twice (maybe three times) a week.  I remember this when I first was on Remeron and though the hunger eased as I got higher, the frozen metabolism did not. 

 

I spoke about it at an Alanon women's meeting and it felt good to share the frustration of having tried many things during this process and now all I can do is wait.

Hi Hibari! I just wanted to check in and see how you are. I'm much better now myself, just the odd day here and there where I feel a bit bleh...I've read your latest posts and I have to tell you that you are an inspiration to me and I will soon be following in your footsteps. We are so alike what with the menopause thing and that big whammy on top of everything else. I want to taper very slowly and cautiously like you are doing. I will continue to be rooting for you and I'm so glad each time you reach your stability. When I have the odd bleh day it makes me forget the many good days I had before it, it seems very strange that way.

I was on Mirtazapine 30 mg for 2.5 years because of severe debilitating menopause related depression and physical symptoms. I tapered down to zero in 2.5 months. Five weeks off on November 10, I started having severe withdrawal symptoms. Took small reinstatement and continued updosing until 30 mg Nov. 30, 2015 as nothing seemed to be getting better and I was impatient. Made a desperate mistake and tried two other ssri's for eight days, bad side effects and stopped. Went back to 30 mg of Mirtazapine and nothing else as of January 14, 2016. Slowly started getting better. After two months symptoms became manageable and continue to slowly fade.

I also take 1.25 mg Premarin/Climara patch for menopause symptoms and have to wean off that too.

Thank you for posting on my thread and for rooting for me.  I'm rooting for you too and I know you can do it.    Yes, menopause has been really tough for me but I think it's because I was so depleted when I started going into it.  My adrenals were low (and probably still are) and I was so tired from caretaking my mom.  Even though I still flash occasionally, the ongoing mood stuff has gone away and my sweating now is from wd.  I've come down on my progesterone, only using about 100mgs and still using the estrogen patch but leave it on for 5 days rather than 3. 

 

I understand about how the bleh day makes you forget all the good days.  I have that too though I find I am able to move through it having had the experience of stabilizing after icky wd symptoms.  I think Alto said, that sometimes there is no avoiding wd symptoms no matter how slow we taper. 

I haven't avoided them and now that they are so familiar that I know it's not because I'm tapering to fast or tapering two drugs at once-it's just because I am weaning off.
